- – service includes support, supervision and engaging participant with eating, bathing, dressing, personal hygiene and other activities of daily living.
- – provides periodic relief for the family or primary caregiver of the individual with developmental disabilities, can be provided in the individual’s home or out of home setting
- – provides assistance with choosing, acquiring and maintaining a job for individuals whom competitive employment has not been achieved or has been interrupted or intermittent. Included pre-job training to prepare the participant, job shadowing, resume preparation, interview skills, study skills and learning skills for job retention.
- – is a habilitation service, it enables the individual to acquire and maintain skills that will allow him/her to function with greater independence in the community. Provides habilitation, training and instruction coupled with elements of support, supervision and engaging participation to reflect the natural flow of training, practice of skills and other activities as they occur during the course of the participants day.
- – this service meets the habilitation and personal care needs of adults (18 years or older) who choose to live with their family and receive supports from their family and whose family wishes to provide services to the participant in the family home. This service is distinctive in that it includes habilitation and training activities, as well as care and assistance with activities of daily living when the participant is dependent on others to ensure health and safety.
- – provides assistance with acquisition, retention, or improvement in skills related to activities of daily living, such as personal grooming and cleanliness, bed making and household chores, eating and the preparation of food and the social and adaptive skills necessary to enable the individual to reside in a non-institutional setting. Habilitation, training and instruction are coupled with elements of support, supervision and engaging participant to reflect the natural flow of training, practice of skills and other activities as they occur during the course of the person’s day.
- – we have the ability to contract with the local school systems to provide one-on-one assistance with students that need that extra assistance or guidance through out their school day to keep them on track and focused on what they should be doing, as well as monitor and deter disruptive behaviors as they may occur
